Liability Waiver:
I hereby certify that my child(ren) is/are in good physical condition and do/does not suffer from any disability that prevents or limits his/her participation in all activities conducted by Brenham Next. I acknowledge that Brenham Next will not assume any responsibility or liability for personal injury or damages caused by the injury.
I hereby waive my right to sue Brenham Next, its owners, directors, officers, agents, and staff.
In the event Brenham Next is unable to reach a parent, guardian or any emergency contact, I hereby give permission for my child(ren) to be transported to the nearest hospital for treatment in case of an accident or emergency. I hereby further authorize(s) any of the staff or employees to provide for, approve, and authorize health care at the hospital.
This Agreement pertains to visits to any activity or program of "Brenham Next" occurring at any time during the duration of BLAST 2025 - 2026, from the date this agreement is signed. Visits and participation in activities and programs thereafter, if any, will be the subject of another agreement. Rights under this agreement, as to prior activities, will not be affected in any way by the new agreement.
Media Release:
I hereby grant and authorize Brenham Next the right to take, edit, copy, publish, distribute and make use of any and all pictures or video taken of my child(ren) to be used in and/or for legally promotional materials and digital communications. This authorization shall continue indefinitely, unless I otherwise revoke said authorization in writing. I understand and agree that these materials shall become the property of and will not be returned.
By Checking the Box you are stating: "I am the parent or legal guardian of the Participant. I have read this document, and I am signing it freely. I understand the legal consequences of signing this document, including (a) releasing Brenham Next, its owners, directors, officers, agents, and staff from all liability on my and the Participant's behalf, (b) waiving my and the Participants' right to sue Brenham Next, its owners, directors, officers, agents, and staff, (c) and assuming all risks of the Participant's participation in this program. I allow the Participant to participate in this program with Brenham Next. I understand that I am responsible for the obligations and acts of the Participant while participating in this program with Brenham Next. I agree to be bound by the terms of this document. "
